ABSTRACT

Abstract The present study evaluated the actual rotational speed of three different endodontic motors compared to the values provided by the manufacturers. A total of three endodontic motors (X-Smart Plus, VDW.Silver, and iRoot) were tested at 400 rpm and 800 rpm and 2 N/cm2 torque. The kinematics of the devices was recorded by using a custom angle-measuring disc with a 50-mm diameter attached to the handpiece provided by the manufacturer, whereas their movement was captured by a high-speed camera at 2,400 frames per second, 800 x 800 pixel-resolution and distance of 0.3 m from the target object. Statistical analysis was performed at a significance level of 5%. At 400 rpm, the iRoot motor had a value of 17.94 rpm above that indicated by the manufacturer, which was significantly different from those of X-Smart Plus (5.20 rpm below that indicated by the manufacturer) and VDW.Silver (0.62 rpm above that indicated by the manufacturer) motors (P < 0.05). At 800 rpm, the iRoot motor had a value of 51.34 rpm below that indicated by the manufacturer, whereas the X-Smart Plus motor had a value of 13.00 rpm below that indicated by the manufacturer (P > 0.05). The VDW.Silver motor statistically differed from the iRoot and X-Smart Plus ones, showing a value of 1.68 rpm above that indicated by the manufacturer. In conclusion, the X-Smart Plus, VDW.Silver, and iRoot motors showed lower variations in the rotational speed values compared to those reported by their manufacturers. The endodontic motors presented different behaviors between them, with the VDW.Silver motor presents the most accurate values and the iRoot presents the most divergent values.


Resumo O presente estudo avaliou a real velocidade rotacional de três diferentes motores endodônticos comparados aos valores fornecidos pelos fabricantes. Um total de três motores endodônticos (X-Smart Plus, VDW.Silver e iRoot) foram testados a 400 rpm e 800 rpm e torque de 2 N/cm2. A cinemática dos dispositivos foi verificada utilizando-se um disco customizado de 50 mm de diâmetro acoplado ao contra ângulo fornecido pelo fabricante e o movimento foi capturado através de uma câmera ajustada em 2400 quadros por segundo com resolução de 800 x 800 pixels distante 0,3 m do objeto alvo. A análise estatística foi realizada com nível de significância de 5%. A 400 rpm, o motor iRoot apresentou 17,94 rpm acima do indicado pelo fabricante, sendo o valor significativamente diferente do X-Smart Plus (5,20 rpm abaixo do indicado pelo fabricante) e VDW.Silver (0.62 rpm acima do indicado pelo fabricante) (P > 0,05). A 800 rpm, iRoot apresentou 51.34 rpm abaixo do indicado pelo fabricante e X-Smart Plus 13,00 rpm abaixo do indicado pelo fabricante (P > 0,05). VDW.Silver foi estatisticamente diferente de iRoot e X-Smart Plus, apresentando 1,68 rpm acima do indicado pelo fabricante. Concluiu-se que os motores X-Smart Plus, VDW.Silver e iRoot apresentaram baixa variação na velocidade rotacional quando comparada ao fornecido pelo fabricante. Os motores endodônticos apresentaram diferentes comportamentos entre eles, sendo que o motor VDW.Silver apresentou valores mais acurados enquanto o iRoot foi o mais divergente.

ABSTRACT

Aim: This study aimed to assess the shaping ability of Reciproc Blue in the apical third and apical foramen of moderately curved canals at different working lengths (WLs), by micro-computed tomography. Methods: Thirty-six mesial roots (mesiobuccal and mesiolingual canals) were included, each with 2 separate root canals and independent apical foramina, according to type IV of Vertucci's classification of first and second mandibular molars. The canals were instrumented at three different WLs: G-1, 1mm short of the major apical foramen; G0, at the major apical foramen; G+1, 1mm beyond the major apical foramen. The groups were assessed for changes in root canal volume and untouched wall area in the apical third. Groups G0 and G+1 were also compared for percentage of untouched walls at the apical foramen. One-way ANOVA (post hoc Tukey test) and Student's t-test adopted a 5% level of significance. Results: Root canal volumes (mm3) in the apical third were 22.86±10.46, 44.48±24.91, and 55.71±21.32 in G-1, G0 and G+1, respectively. G-1 volume following instrumentation increased significantly less than that of G0 or G+1 (P>.05); G0 did not differ from G+1. The percentage of untouched wall area in the apical third did not differ among the three groups (P>.05). G0 and G+1 did not differ regarding untouched walls in the major apical foramem walls. Conclusion: Extending the WL from 1mm short of the apical foramen to a point at and beyond the WL increases the apical third volume without increasing the prepared area. Untouched surface areas of the apical foramen were not modified by instrumentation at or beyond the foramen

ABSTRACT

Abstract This study assessed 3 endodontic motors, X-Smart Plus (Dentsply Sirona, Ballaigues, Switzerland), VDW.Silver Reciproc (VDW GmbH, München, Germany) and, iRoot (Bassi Endodontics, Belo Horizonte, Brazil) in 2 different reciprocating settings. The movements evaluated were 170° in counter-clockwise (CCW) and 50° in clockwise (CW) at 350 RPM, and 150° CCW and 30° CW at 300 RPM. For the X-Smart Plus and VDW Silver the settings used were the ones in the motor library. For the iRoot, the motor was adjusted to the angles of the study. A customized optic target was attached to the contra-angle of the motor and the movements were recorded with a high-resolution camera (K2 DistaMaxTM Long-Distance Microscope System, Infinity Photo-Optical Company, Colorado, EUA) at 2,400 frames per second (FPS). The images were analyzed with the Vision Research software (Inc. Headquarters, Wayne, New Jersey, EUA). The following kinematic parameters were assessed: CCW angle, CW angle, speed (RPM) at both directions, and, standstill time at each change of directions. The Intraclass Correlation Coefficient (ICC) and Kruskal-Wallis (method of Dunn) were used at a significant level of 5%. There was no statistically significant difference among the motors at the 150°/30° setting (P > .05); the iRoot was the least reliable at the 170°/50° setting for CCW angle, speed, and net angle parameters (P < 0.05). The standstill time of all motors in both directions was identical. None of the motors were able to reproduce faithfully the set movements. The iRoot motor presented a higher discrepancy when compared to X-Smart and VDW Silver.


Resumo Este estudo avaliou 3 motores endodônticos, X-Smart Plus (Dentsply Sirona, Ballaigues, Suíça), VDW.Silver Reciproc (VDW GmbH, Munique, Alemanha) e iRoot (Bassi Endodontics, Belo Horizonte, Brasil) em 2 diferentes ajustes de movimento reciprocante. Os movimentos analisados foram: 170° em sentido anti-horário (CCW) e 50° em sentido horário (CW) a 350 RPM, e 150° CCW e 30° CW a 300 RPM. Para os motores X-Smart Plus e VDW Silver os ajustes usados foram os que se apresentam nos ajustes pré-definidos dos motores. Para o iRoot o motor foi ajustado para os ângulos do estudo. Um alvo ótico customizado foi preso ao contra ângulo do motor e os movimentos foram gravados com uma câmara de alto resolução (K2 DistaMaxTM Long-Distance Microscope System, Infinity Photo-Optical Company, Colorado, EUA) a 2.400 quadros por segundo (FPS). As imagens foram analisadas com o sotware Vision Research (Inc. Headquarters, Wayne, Nova Jersey, USA). Os seguintes parâmetros de cinemática foram avaliados: ângulo anti-horário, ângulo horário, ângulo líquido, velocidade (RPM) em ambas direções e tempo de parada a cada mudança de direção. O Coeficiente de Correlação Intraclasse (ICC) e o teste de Kruskal-Wallis (método de Dunn) foram usados com nível de significâncias de 5%. Não houve diferença estatisticamente significante entre os motores no ajuste de 150°/30° (P > .05); o motor iRoot foi o menos confiável no ajuste de 170°/50° para o ângulo anti-horário, velocidade e ângulo líquido (P < 0.05). O tempo de parada em todos os motores foi idêntico em ambas as direções. Nenhum dos motores foi capaz de reproduzir fielmente os movimentos. O motor iRoot apresentou maior discrepância quando comparado com o X-Smart Plus e VDW Silver.

ABSTRACT

Aim: The aim of this study was to evaluate the effect of ethanol-conditioned dentin on endodontic sealer penetration into dentinal tubules by confocal laser scanning microscopy (CLSM). Methods: Forty human maxillary anterior teeth were instrumented and divided into four groups (n = 10) according to the drying methods: 1) wet: vacuum only, 2) paper points: vacuum + absorbent paper points, (3) 70% ethanol: 70% ethanol (1 min) + vacuum + absorbent paper points, and (4) 100% ethanol: 100% ethanol (1 min) + vacuum + absorbent paper points. All root canals were filled with resin-based endodontic sealer. Four sections from each third (cervical, middle, and apical) were examined by CLSM. Root canal wall perimeter infiltrated by sealer, maximum depth of sealer penetration, percentage of penetrated area, and fluorescence intensity of rhodamine B were measured. Statistical analysis was performed by analysis of variance and Tukey's tests (α = 0.05). Results: No statistical difference was found when percentage of root canal wall coverage infiltrated by sealer were compared. The groups in which ethanol solutions were used presented greater depth of sealer penetration, higher percentage of penetrated area, and higher fluorescence intensity of rhodamine B (p< 0.05) when compared with the wet and paper point groups. Overall, 100% ethanol produced better results than 70% ethanol, except for rhodamine B intensity (cervical third). In addition, the absorbent paper points drying method behaved better than did vacuum only group, except for rhodamine B intensity (apical third). Conclusion: Ethanol-conditioned dentin improved the penetration of resin-based sealer into dentinal tubules, especially at the concentration of 100%

Objetivo: Avaliar, por meio de tomografia computadorizada de feixe cônico (TCFC), a morfologia dos canais radiculares de dentes incisivos inferiores, em uma população da região Sudeste brasileira, e sua relação com a idade e o sexo dos pacientes Métodos: Foram analisados 371 prontuários de pacientes de ambos os sexos que continham exames de TCFC, totalizando 1.484 dentes incisivos inferiores. As imagens tomográficas foram capturadas com o tomógrafo Orthopantomograph OP300, com voxel de 0,20mm. A morfologia dos dentes foi avaliada de acordo com a classificação de Vertucci, e foram investigados os efeitos do sexo e da idade dos pacientes sobre a variação na morfologia desses dentes. Resultados: Todos os dentes avaliados apresentaram apenas uma raiz; 80,7% dos incisivos inferiores apresentaram um único canal (Tipo I de Vertucci). O segundo canal esteve presente em 19,3% dos casos, sendo do Tipo II de Vertucci em 1,1%, Tipo III em 18,1%, Tipo IV em 0,1% e Tipo V em 0,1%. Não houve influência estatisticamente significativa (p=0,890) do sexo na morfologia dos incisivos inferiores. Dentro da faixa etária analisada, os indivíduos com menos de 18 anos e aqueles com 40 a 49 anos (28,2% e 26,8% respectivamente) apresentaram significância estatística (p=0,001) quanto à presença de segundo canal, compa- rados aos de outras faixas etárias. Conclusões: A morfologia mais prevalente foi a Tipo I de Vertucci, seguida pelo Tipo III, sem diferença entre os sexos, e com maior prevalência de variação morfológica nos indivíduos com menos de 18 anos e com 40 a 49 anos de idade (AU).


Objective: Evaluate the root canal morphology of per- manent mandibular incisors, through the use of cone bean computed tomography (CBCT) in a population of Brazilian southeast region and its relationship with patient age and sex. Methods: A total of 1,484 mandibular incisors of 371 male and female patient`s medical records were analyzed using CBCT. The tomographic images were by the orthopantomograph OP300 tomograph, with voxel of 0.20 mm. The morphology of the teeth was evaluated according to the classification of VERTUCCI, and the effects of sex and age of the patients on the variation in the morphology of these teeth were investigated. Results: All evaluated teeth had only one root. A single canal (Ver- tuccis type I) was detected in 80.7% of mandibular incisors. The second canal was present in 19.3% of the cases, with rates of 1.1% for Type II, 18.1% for Type III, 0.1% for Type IV, and 0.1% for Type V. According to sex, there was no statistical difference (p = 0.890) in mandibular incisor morphology. Within the analyzed age group, individuals aged under 18 years and individuals between 40 and 49 years (28.2% and 26,8% respectively) were statistically significant (p = 0.001) when compared to the other age groups. Conclusions: The most prevalent morphology was Type I of VERTUCCI, followed by Type III, without difference between sex and with a higher prevalence of morphological variation in individuals with less than 18 years and between 40 and 49 years (AU).

ABSTRACT

The aim of this study was to evaluate apical transportation and apical root canal sealing after root canal filling in human teeth prepared with MTwo® Rotary System with and without apical foramen enlargement. Twenty mandibular premolars were divided into two groups (n=10). Group 1 had root canals prepared 1mm beyond the apical foramen. Group 2 had root canals prepared 1mm below the root canal length. After chemo-mechanical preparation, samples were submitted to scanning electronic microscopy. Apical foramen images had 75x magnification at standardized positions, allowing measurements from the apical foramen area before and after root canal preparation, and after root canal filling. Apical foramen shape and apical transportation, as well as its level of circumferential filling after root canal preparation were accessed using the Image Subtraction System. Scanning electronic microscopy analysis demonstrated that samples of Group 1 showed larger foraminal diameter than samples of Group 2 (p<0.05). Apical foramen transportation was statistically different between Groups 1 and 2 (p=0.0108). Furthermore, the apical foramen sealing also differed statistically between groups 1 and 2 (p=0.0007) and 100% of samples of Group 1 showed apical root canal sealing. Apical root canal sealing was more effective when the root canal was prepared with apical foramen enlargement, even when the apical transportation was detected.

ABSTRACT

Objective: to evaluate the root canal treatment (RCT) and non-surgical root canal retreatment (NSRCR), associated with foraminal enlargement, performed on a singlevisit. Material and Methods: 125 teeth with apical periodontitis and follow-up period ranging from 6 to 12 months were included. The success was considered by the absence of signs and symptoms and complete or incomplete periapical repair. Logistic regression analyses were used to identify factors associated with the repair (p<0.05). Results: RCT showed 71.58% of complete healing and 23.16% of acceptable healing. NSRCR showed 80% of complete healing and 20% of acceptable healing. Age, gender, type of treatment and preoperative pain were not statistically significant for the healing process (p>0.05). Premolars showed the greatest chance of periapical repair. Pulp Canal Sealer showed a greater chance of periapical repair when compared to Sealapex (p<0.05). Conclusion: RCT and NSRCR using a foraminal enlargement protocol provided a favorable prognosis of periapical healing


Objetivo: avaliar o tratamento endodôntico (TE) e o retratamento não cirúrgico do canal radicular (RNCCR), associado a ampliação foraminal, realizado em sessão única. Material e Métodos: foram incluídos 125 dentes com periodontite apical e proservação de 6 a 12 meses. O sucesso foi considerado pela ausência de sinais e sintomas e reparo periapical completo ou incompleto. Análises de regressão logística foram utilizadas para identificar fatores associados ao reparo (p<0,05). Resultados: o TE mostrou 71,58% de cicatrização completa e 23,16% de cicatrização aceitável. O RNCCR mostrou 80% e 20% cicatrização completa e aceitável, respectivamente. Idade, sexo, tipo de tratamento e dor pré-operatória não foram estatisticamente significantes para o processo de cicatrização (p>0,05). Os pré-molares apresentaram a maior chance de reparo periapical. O cimento Pulp Canal Sealer mostrou maior chance de reparo periapical quando comparado ao Sealapex (p<0,05). Conclusão: o TE e RNCCR utilizando um protocolo de alargamento foraminal, proporcionaram um prognóstico favorável da cicatrização periapical

ABSTRACT

The coronavirus outbreak (2019) represents a public health emergency of global concern. Several measures have been taken to minimise the risk of infection among the population, including social distancing, working from home, closure of non-essential activities since the detection of the first case in Brazil. This study describes the teaching experiences during the SARS-COV-2 pandemic in a Brazilian School of Dentistry. The State University of Campinas (UNICAMP) was the first public university in Brazil to stop all the classroom activities on 13th of March 2020 due to COVID-19, followed by other universities. UNICAMP developed several initiatives and created a special support page for digital teaching, where it is possible to obtain guidance, support materials for teachers and a space for exchanging messages (e-mail and chat) for specific guidelines. UNICAMP has started lending computer equipment to undergraduate and graduate students according to socioeconomic criteria. Along with the equipment, 500 chips have been delivered with 10 GB of internet so that these students are able to access the network and carry out the remote activities related to their courses. In conclusion, quality education is the key-element in forming high-quality professionals that will in a near future provide health care for the community, be part in international research groups and become lecturers

Abstract The objective of this study was to assess the influence of chlorhexidine (liquid and gel) and zinc oxide in calcium hydroxide (CH) pastes on root pH in simulated external resorption. One hundred human anterior teeth with a single root canal were selected. After decoronation and root canal instrumentation, the specimens were divided into 4 experimental groups and 1 control group (without intracanal paste): CH + saline (CH+S), CH + 2% chlorhexidine liquid (CH+ CHX), CH + 2% chlorhexidine gel (CH+ CHXg), and CH + 2% chlorhexidine gel + zinc oxide (CH+ CHXg+ZnO). pH was measured using a microelectrode at 3 and 24 h, and 1, 2, 3, and 4 weeks after inserting intracanal pastes. Data were analyzed statistically using an ANOVA and Tukey's test (p < 0.05). The CH+CHXg+ZnO group had the highest pH values throughout (p<0.05). The CH+S and CH+ CHX groups had the highest pH values after 1 week and the CH+ CHXg group after 2 weeks. CH+ CHXg maintained the highest pH until the fourth week compared with CH+ CHX (p < 0.05). The control group remained at a neutral pH at all evaluated times. It can be concluded that chlorhexidine solution or gel maintained the alkaline pH of CH, and chlorhexidine gel allowed a slower decrease in pH over time. CH+ CHXg+ZnO showed the highest pH values and was an effective intracanal medication for maintaining alkaline root pH in the area of resorption.

ABSTRACT Objective: This retrospective study evaluated the demographic profile of patients and the clinical characteristics of dental emergencies in a brazilian university outpatient clinic. Methods: During the period from June 2013 to May 2014, data were collected from 1.345 clinical records. Chief complaint, gender, age, systemic alteration, clinical inspection of the affected tooth, pulp and periapical diagnosis, the type of procedure performed and whether there was the need for post-operative systemic medication. Data were analyzed using the SAS software and chi-square test was used to verify the presence or absence of statistical significance at a level of 5%. Results: Dental pain (53.3%) was the main reason why people attended the emergency service, that is, female patients (63.1%), aged 29-48 years old (42%), patients without systemic conditions (75%), and patients with decayed teeth (25%). In addition, involvement of lower molars (29.81%) and diagnosis of irreversible pulpitis (30.42%) were the most frequent findings. Emergency endodontic treatment (45.8%) was the most performed procedure. Irreversible pulpitis (30.4%) and pulp necrosis associated with acute apical periodontitis (21.9%) were the most frequent conditions requiring emergency endodontic treatment, and in most cases, there was no need for post-operative systemic medications (95.7%). Conclusion: The prevalence of endodontic emergencies during the period of one year emphasizes the need for more accurate diagnosis and the importance of providing this service to the population.


RESUMO Objetivo: Este estudo retrospectivo avaliou o perfil demográfico dos pacientes e as características clínicas das urgências dentárias em um Serviço de Plantão de Urgências de uma Faculdade brasileira de Odontologia. Métodos: Durante o período de junho de 2013 a maio de 2014, os dados foram coletados de 1.345 registros clínicos, com informações sobre a queixa principal, gênero, idade, alteração sistêmica, inspeção clínica do dente afetado, diagnóstico pulpar e periapical, o tipo de procedimento realizado e se houve necessidade de medicação sistêmica pós-operatória. Os dados foram analisados usando o software SAS e o teste do qui-quadrado foi utilizado para verificar a presença ou ausência de significância estatística ao nível de 5%. Resultados: A dor dental (53,3%) foi a queixa principal mais encontrada. Pacientes do gênero feminino (63,1%), com idade entre 29-48 anos (42%), sem condições sistêmicas (75%) e com dentes cariados (25%) foram os mais prevalentes. Além disso, o envolvimento de molares inferiores (29,81%) e o diagnóstico de pulpite irreversível (30,42%) foram os achados mais frequentes. O tratamento endodôntico de urgência (45,8%) foi o procedimento mais realizado. A pulpite irreversível (30,4%) e a necrose pulpar associada a periodontite apical aguda (21,9%) foram as condições mais frequentes que necessitaram de tratamento endodôntico de urgência e, na maioria dos casos, não houve prescrição de medicamentos sistêmicos pós-operatórios (95,7%). Conclusão: A prevalência de urgências endodônticas durante o período de um ano enfatiza a necessidade de diagnósticos mais precisos e a importância de oferecer esse serviço à população.

ABSTRACT

Abstract This study aimed to evaluate the influence of different ethanol concentrations on dentin roughness, surface free energy, and contact angle between AH Plus and the root canal dentin. One hundred human maxillary anterior teeth were split longitudinally and 200 dentin specimens were polished to make the surface flatter and smoother. An acrylic bar was positioned between two dentin specimens and impression material was added to create a block, simulating an instrumented root canal space. Specimens were removed from the mold and cleaned in an ultrasonic bath for 10 min. Thereafter, dentin specimens were divided into four groups (n = 50) according to the drying methods used: a) wet: vacuum only, b) paper points: vacuum + absorbent paper points, c) 70% alcohol: 70% alcohol (1 min) + vacuum + absorbent paper points, and d) 100% alcohol: 100% alcohol (1 min) + vacuum + absorbent paper points. A rugosimeter and a goniometer were used to verify the roughness (Ra) and to measure the surface free energy and the contact angle between the AH Plus sealer and the root canal dentin. ANOVA and Tukey tests (α = 0.05) were used for statistical analysis. The 70% and 100% ethanol groups showed significantly decreased roughness as well as increased surface free energy in the root canal dentin when compared to the wet and paper point groups. In addition, ethanol significantly reduced the contact angle between the AH Plus sealer and the root canal dentin. Ethanol solutions (70% and 100%) provide better wettability of AH Plus sealer on dentin surfaces.

ABSTRACT

Abstract This clinical study aimed to evaluate the relationship of the delay between dental trauma and the initial attendance to the development of external inflammatory root resorption in permanent teeth affected by severe luxation. Sixty-seven patients, aged between 11 and 56 years, presenting 133 injured teeth with closed apex (56 extrusive luxation, 69 lateral luxation and 8 intrusive luxation) were followed up for a minimum of 24 months. The time elapsed between dental trauma and the initial attendance was recorded. The presence of necrotic pulp and external inflammatory resorption for each type of trauma was verified. Fisher’s exact test was used to verify the influence of the initial attendance date at the Dental Trauma Center and the development of inflammatory resorption (p<0.05). The odds ratio was used to check the risk of developing external inflammatory resorption due to delay in seeking treatment. Pulp necrosis was observed in 105 teeth (78.9%) and external inflammatory resorption was detected in 17.8% cases of extrusive luxation (10 teeth), 15.9% of lateral luxation (11 teeth) and 25% of intrusive luxation (2 teeth). For lateral luxation, extended delay between the date of injury and initial attendance influenced the occurrence of external inflammatory resorption (p=0.0174). Patients who began treatment 45 days after the injury were 3.4 times more likely to develop external inflammatory resorption than patients who sought treatment after the trauma. Treatment late after the occurrence of dental trauma can impair the prognosis and result in the development of external inflammatory resorption in luxated teeth.


Resumo O objetivo deste estudo clínico foi avaliar a relação entre a demora na procura por tratamento e o desenvolvimento de reabsorções inflamatórias externas em dentes luxados com ápice fechado. A amostra desta pesquisa foi constituída por 67 pacientes que apresentaram 133 dentes traumatizados, dos quais 56 foram acometidos por luxação extrusiva, 69 por luxação lateral e 8 por luxação intrusiva, proservados por no mínimo 24 meses. Foi registrado o tempo decorrido entre a data do trauma e o primeiro atendimento. Verificou-se a ocorrência de necrose pulpar e reabsorções inflamatórias externas em cada tipo de traumatismo. O Teste Exato de Fisher foi aplicado para verificar a influência da data do atendimento inicial e o desenvolvimento de reabsorções inflamatórias externas em cada tipo de luxação e o teste de Odds Ratio foi aplicado para verificar o risco do desenvolvimento de reabsorções inflamatórias externas em função da demora na procura por tratamento. Os resultados revelaram que a necrose pulpar foi verificada em 105 dentes luxados (78,9%) e a reabsorção inflamatória externa foi verificada em 17.8% das luxações extrusivas (10), sendo 15,9% das luxações laterais (11) e 25% das luxações intrusivas (2). A demora na procura por tratamento influenciou o desenvolvimento de reabsorções infamatórias externas nas luxações laterais (p=0,0174). Os pacientes que procuraram tratamento após 45 dias da data do trauma apresentaram 3,4 vezes mais chance de desenvolver reabsorção inflamatória externa do que os pacientes que procuraram tratamento logo após o traumatismo. Conclui-se que o tratamento tardio após a ocorrência do traumatismo pode prejudicar o prognóstico e favorecer o desenvolvimento de reabsorções inflamatórias externas em dentes luxados com ápice fechado.

ABSTRACT

La necrosis pulpar en dientes con ápice abierto representa un desafio al tratamiento endodóntico. Esto debido a que la anatomía radicular se encuentra parcialmente formada, con ápices abiertos, paredes radiculares finas y frágiles, así como la longitud radicular reducida. En estas situaciones, cuatro opciones de tratamiento están disponibles: 1) Apexificación con renovación periódica de medicación intraconducto (MIC), hasta que sea posible observarradiográficamente el cierre apical; 2) Apexificación sin renovación de MIC; 3) Confección de una barrera apical utilizando agregado de trióxido mineral (MTA) o Biodentine; 4) Revascularización pulpar. El endodoncista debe estar atento a la evolución tanto de las técnicas de tratamiento como de los materiales dentales, conociendo sus indicaciones y limitaciones. Este artículo llevó a cabo una revisión de la literatura sobre las diferentes formas de tratamiento en casos de dientes traumatizados con rizogénesis incompleta y necesidad de tratamiento endodóntico por necrosis pulpar, indicando las ventajas, desventajas y limitaciones de cada una.


Necrotic pulp of teeth with open apex leads to challenges in endodontic treatment because the anatomy of the root canal is incompletely formed with widely open apex, root walls thin and fragile and reduced root length. In these situations, four treatment options are available: 1) Apexification with periodic exchanges of intracanal medication (ICM), until radiographic evidence of apical closure; 2) Apexification without exchanges of ICM; 3) Apical barrier using mineral-trioxide-aggregate (MTA) or Biodentine; 4) Pulp revascularization. The endodontist should be attentive to the evolution of treatment techniques and dental materials, with their limitations and directions. This article reviews the literature on the different forms of treatment in cases of traumatized immature teeth, indicating the advantages, disadvantages and limitations of each one.

ABSTRACT

Objetivo: verificar a frequência dos atendimentos realizados no Serviço de Traumatismos Dentários da FOP- -Unicamp, durante o período de fevereiro de 2013 a fevereiro de 2015. Sujeitos e método: os dados foram coletados na ficha clínica utilizada no serviço e foram avaliados os seguintes aspectos: gênero, faixa etária, classificação do dente, período entre o trauma e o primeiro atendimento no serviço, etiologia, classificação da injúria traumática e procedimento efetuado. Os dados foram analisados com auxílio do software SAS e do teste Qui-Quadrado, que foi utilizado para verificar a presença ou a ausência de significância com nível de 5%. Resultados: foram analisadas as fichas clínicas de 57 pacientes com 161 dentes traumatizados, com maior prevalência do gênero masculino e da faixa etária abaixo dos 14 anos. Os incisivos centrais (60,9%) foram os dentes mais afetados. A maioria dos pacientes (56,1%) procurou atendimento entre 0 e 30 dias após o trauma. As quedas (33,3%) foram a etiologia mais comum. As injúrias dentárias traumáticas mais encontradas foram as fraturas coronárias de esmalte e dentina (19,9%) e a subluxação (19,3%). O procedimento mais efetuado foi a realização dos exames clínico e radiográfico (46,6%). Conclusão: os achados mais prevalentes dos atendimentos foram: o gênero masculino; a maior frequência de injúrias na faixa etária abaixo dos 14 anos de idade; os incisivos centrais como os dentes mais acometidos; o período entre 0 e 30 dias para procurar atendimento; as quedas, a etiologia mais comum; os exames clínico e radiográfico, o procedimento mais efetuado; as fraturas coronárias de esmalte e dentina e a subluxação, as injúrias mais observadas.

Objective: This study investigated the short-term subcutaneous tissue reaction of a new endodontic paste, based on the association among calcium hydroxide [Ca(OH)2], 2% chlorhexidine gel (CHX) and zinc oxide (ZnO) in comparison with a control paste, and the individual substances alone. Material and method: Twenty-four male wistar rats were used. Polyethylene tubes containing the materials were implanted into the dorsal connective tissue of rats. Empty implanted tubes served as controls. The materials used were: Ca(OH)2+CHX+ZnO paste in two different proportions (2:1:2) and (2:1:3), a control paste, Ca(OH)2+CHX (2:1), and the substances alone. After time intervals of 2, 3, 7 and 15 days, the animals were euthanized and the specimens prepared for histological processing. Data were evaluated qualitatively according to the levels of tissue repair and the degree of inflammation. Result: On the second day all the materials were observed to cause a severe inflammatory response. On the third day, the pastes Ca(OH)2+CHX+ZnO (2:1:2) and (2:1:3) showed reduction in degree of inflammation. On seventh day the tissue was already more organized with presence of collagen fibers in all materials. After 15 days it was found that compound Ca(OH)2+CHX+ZnO (2:1:2) showed areas with no inflammatory infiltrate. Conclusion: The experimental pastes, Ca(OH)2+CHX+ZnO in two different proportions (2:1:2) and (2:1:3), showed favorable results with respect to biocompatibility, when evaluated for a short-term.


Objetivo: O presente estudo investigou a reação em tecido subcutâneo de ratos, por um curto período, de uma nova pasta endodôntica, obtida a partir da associação entre o hidróxido de cálcio [Ca(OH)2], clorexidina gel 2% (CHX) e óxido de zinco (ZnO), em comparação à pasta controle, e as substâncias isoladamente. Material e método: Foram utilizados vinte e quatro ratos wistar machos. Tubos de polietileno contendo os materiais foram implantados no tecido conjuntivo dorsal de ratos. Tubos vazios foram utilizados como controle. Os materiais utilizados foram: Pasta de Ca(OH)2 + CHX + ZnO em duas proporções diferentes (2: 1: 2) e (2: 1: 3), pasta controle, Ca(OH)2< + CHX (2:1), e as substâncias isoladas. Após intervalos de tempo de 2, 3, 7 e 15 dias, os animais foram sacrificados e as amostras preparadas para processamento histológico. Os dados foram avaliados qualitativamente de acordo com os níveis de reparo dos tecidos e do grau de inflamação. Resultado: No segundo dia observou-se que todos os materiais causaram uma resposta inflamatória grave. No terceiro dia, as pastas de Ca(OH)2 + CHX + ZnO (2: 1: 2) e (2: 1: 3) mostraram uma redução no grau de inflamação. No sétimo dia, o tecido encontrava-se mais organizado, com a presença de fibras colágenas em todos os materiais. Após 15 dias, verificou-se que a pasta de Ca(OH)2 + CHX + ZnO (2: 1: 2) mostrou áreas sem infiltrado inflamatório. Conclusão: As pastas experimentais, Ca(OH)2 + CHX + ZnO, nas duas proporções avaliadas (2: 1: 2) e (2: 1: 3), mostraram resultados favoráveis em relação à biocompatibilidade, quando avaliada por curto período de tempo.

Aim: This prospective randomized clinical study examined the influence of two different auxiliary chemical substances on postoperative pain in 301 single-visit endodontic treatments, with enlargement of the apical foramen and extrusion of cement into the periapical region. Material and method: The two auxiliary chemicals used were 2% chlorhexidine (2% CHX gel; n = 145) and 5.25% sodium hypochlorite (5.25% NaOCl; n = 156). The incidence of postoperative pain and discomfort was assessed at 24 hours and expressed as percentages. The Fisher exact test and the Chi-square test were used to compare variation in postoperative pain. The variables analyzed were previous pain, pulp status, age, and number of root canals. Result: In teeth with previous pain instrumented with 2% CHX gel, the incidence of postoperative pain was 22.22% (6/27) versus 11.11% (3/22) in teeth instrumented with 5.25% NaOCl. In teeth without previous pain instrumented with 2% CHX gel, the incidence of postoperative pain was 8.5% (6/118) versus 2.33% (3/129) in teeth instrumented with 5.25% NaOCl, with no statistically significant difference between the groups. Results showed that previous pain had a significant influence on postoperative status (p <0.001). After 24 hours postoperatively, 93.7% (282/301) of the teeth had no pain and 6.3% (19/301) had some level of pain, and used one or two doses of medication. Conclusion: Based on the results, it can be concluded that the auxiliary chemical substances had no influence on postoperative pain. .


Objetivo: Este estudo clínico prospectivo randomizado analisou a influência de duas substâncias químicas auxiliares, com diferentes potenciais de toxicidade, na dor pós-operatória observada em 301 tratamentos endodônticos concluídos em uma única sessão, com ampliação do forame apical e sobre-extensão de cimento para o periápice. Material e método: Foram usados gel de clorexidina a 2% (CHX 2% gel; n = 145) e hipoclorito de sódio a 5,25% (NaOCl 5,25%; n = 156). A incidência de dor pós-operatória e desconforto foi avaliada em 24 horas, e foi expressa em porcentagem. O teste exato de Fischer e o teste de Qui Quadrado foram utilizados para comparar a variação da dor pós-operatória. Os fatores analisados foram dor prévia, estado pulpar, idade e número de canais radiculares. Resultado: Nos dentes com dor prévia e instrumentados com CHX 2% gel, a incidência de dor pós-operatória foi 22.22% (6/27), contra 11.11% (3/22) nos dentes instrumentados com NaOCl 5,25%. Nos dentes sem dor prévia e instrumentados com CHX 2% gel, a incidência de dor pós-operatória foi 5.08% (6/118), contra 2.33% (3/129) nos dentes instrumentados com NaOCl 5,25%, sem diferenças estatisticamente significativas entre os grupos. Os resultados mostraram que a dor prévia exerceu uma influência significativa no estado pós-operatório (p <0,001). Após 24 horas, 93,7% (282/301) dos dentes não apresentaram dor, ao passo que 6,3% (19/301) tiveram algum nível de dor pós-operatória e fizeram uso de uma ou duas doses da medicação. Conclusão: Diante dos resultados, podemos concluir que a substância química auxiliar não está associada à dor pós-operatória. .

Introduction : Dental trauma care emergency is very important issue, especially in cases of avulsion and it is crucial for the prognosis. A preventive-educational approach had not been effective and there are many failures in handling dental trauma, mainly by lack of knowledge by health professionals, teachers and lay people. Objective: The aim of this study was to assess the knowledge of 89 teachers about dental trauma. Material and methods: A questionnaire divided into three parts containing questions about the emergency procedures in cases of dental trauma was applied. Results: Only 13% of teachers would replant the tooth in the socket, and only 7% said they would put the tooth in some liquid and 58% would store in a piece of paper, cloth or clean container. In relation to replantation, 75% reported that they would hold the tooth by the crown, 79% reported that first they would refer to the dentist, and 80% thought that the treatment had to be immediate. With regard to tooth preparation, 46% would keep it in saline, 24% in water, and only 11% in milk. Concerning to the avulsed tooth, only 15% correctly answered that they would replant the avulsed tooth and then referred to the dentist. Conclusion: It was concluded that the knowledge of teachers must be improved by educational and preventive campaigns on management of traumatized teeth.

ABSTRACT

O objetivo deste estudo clínico prospectivo randomizado foi analisar a influência de duas substâncias quimicas auxiliares na identificação de canais acessórios e no selamento do forame apical pelo cimento endodôntico e a prevalência desses canais na faixa etária de 13 a 79 anos, nos diferentes grupos dentários, em dentes vitais, não vitais e nos retratamentos endodônticos. Trezentos e um tratamentos endodônticos foram concluídos em 240 pacientes. f';s substâncias químicas auxiliares utilizadas foram a clorexidina gel 2% (n= 145) e o hipoclorito de sódio 5.25% (n= 156). Os dentes foram preparados pela técnica coroa-ápice com patência e ampliação do fora me apical e obturados pela técnica de termoplastificação da guta-percha. Três observadores independentes (triplo cego) avaliaram, radiograficamente, o preenchimento de canais acessórios e do forame apical que foi registrado e expresso em dados percentuais. Os dados foram analisados, estatisticamente, e os resultados mostraram que o forame apical foi selado pelo cimento endodôntico em 98.34% (296/301) e os canais acessórios foram preenchidos e visíveis radiograficamente em 47.84% (144/301) dos dentes tratados. Canais acessórios foram encontrados em 55% (99/180) dos dentes multirradiculares e em 37.20% (45/121) dos dentes unirradiculares. De acordo com o estado pulpar, canais acessórios foram encontrados em 55,20% (69/125) dos dentes vitais, 49.00% (49/100) dos dentes não vitais e 34.21% (26/76) dos retratamentos endodônticos. Concluiu-se que, entre os fatores analisados, não houve diferença estatisticamente significativa entre os dois grupos das substâncias químicas auxiliares analisadas.


The aim of this prospective randomized study, conducted in vivo, was to analyze the influence of two auxiliary chemicals in filling accessory canais and apical foramen by sealer and the prevalence of these channels in the age group 13 to 79 years, in various dental groups, in vital and non-vital teeth and root canal retreatment. Two hundred forty patients were treated and 301 endodontic treatments were completed. The auxiliary chemicals used were 2% chlorhexidine gel [n = 145) and 5.25% sodium hypochlorite (n = 156). The teeth were instrumented by crown-down technique with patency and enlargement of the apical foramen and obturated by the thermoplastic gutta-percha technique. Three independent observers (triple blind) evaluated radiographically obturation of accessory canais and apical foramen by the sealer that was recorded and expressed as a percentage. Data were statistically analyzed using the Binominal and Qui Quadrado tests. The results showed that the apical foramen was filled in 98.34% (296/301) and the accessories channels were filled and radiographically visible in 47.84% (144/301) of teeth treated. Accessory canais were found in 55% (99/180) of the teeth multirooted and 37.20% (45/121) of single-rooted teeth. According to the state of pulp, accessory canais were found in 55.20% (69/125) of vital teeth, 49.000/0 (49/100) of non-vital teeth and 34.21% (26/76) of endodontic retreatment. It was concluded that among the factors examined, there was no statistically significant difference between the two groups.

ABSTRACT

Pulp revascularization can be considered as a current alternative treatment to apexification, recommended for immature teeth cases, requiring endodontic treatment. Apexification involves long-term periodic exchanges of a calcium hydroxide paste into the root canal to induce the formation of a calcified barrier. Despite being the most classically therapy employed for these cases, the permanence of calcium hydroxide for long periods of time and also the successive changes may lead to a weakening of the root due to its hygroscopic properties and the proteolytic activities of calcium hydroxide, increasing the risk of fractures and contamination of the pulp space. Thus, a constant search for new treatment alternatives that provide the end of root development have been done to avoid the risk of future root fractures. So, revascularization has emerged as a new treatment option for cases of undeveloped teeth, that provides not only apical closure, as apexification, but also increase the dentin walls thickness. In the literature, there is an assortment of treatment protocols employing pulp revascularization procedure in attempt to attain the best way to achieve success. Assuming the diversity of protocols for revascularization treatment, it is important to go deep in the literature to collect, describe and discuss these protocols guiding new researches in this field and also conducting the clinicians. Therefore, this review aims to assess the literature on the different revascularization protocols.


A revascularização pulpar pode ser considerada atualmente como um tratamento alternativo à apicificação e é indicado para dentes com rizogênese incompleta e necessidade de tratamento endodôntico. A apicificação envolve a realização de trocas periódicas em longo prazo de uma pasta de hidróxido de cálcio com o objetivo de formar uma barreira calcificada. Apesar de ser a terapia mais empregada, a permanência desta medicação por longos períodos e as trocas sucessivas, podem levar à fragilização da raiz devido às propriedades higroscópicas e proteolíticas do hidróxido de cálcio, aumentando o risco de fraturas e contaminação do espaço pulpar. Desta forma, tem-se buscado novas alternativas de tratamento que possibilitem o fechamento do forame apical dos dentes imaturos sem que ocorra a fragilização dos mesmos. A revascularização pulpar tem surgido como uma nova opção de tratamento para estimular o término do desenvolvimento radicular e não apenas o fechamento apical. Há na literatura uma variedade de protocolos utilizando esta técnica, buscando sempre alcançar o sucesso no tratamento. Diante dessa variedade, é de grande importância a realização de uma revisão de literatura sobre revascularização pulpar visando reunir, descrever e discutir os diferentes protocolos, para que possam direcionar novas pesquisas e ser empregada de forma adequada pelo clínico. O objetivo desse trabalho é revisar na literatura os diferentes protocolos de revascularização pulpar.
